Brad Mayhew served as a wildland firefighter with the U.S. Forest Service Los Padres Hotshots and Santa Ynez Flight Crew, and has an M.S. in Human Factors and System Safety.

Since 2005, he has offered workshops on organizational learning, risk and resilience, and other Human Factors topics. These courses have been delivered to a variety of organizations, and the courses range from executive education to front-line training.

He helped author the Human Factors pages in the Incident Response Pocket Guide (National Wildfire Coordinating Group, 2006), and his work has appeared in the annual WFSTAR Videos (2006, 2010) and FireRescue Magazine.  In 2007, he received the National Wildfire Coordinating Group Leadership Committee’s “Paul Gleason Lead by Example Award” for Innovation.

He has served various roles on accident investigation teams.
